I recently moved to Ontario and don't have a valid Ontario Health Card. What do I do?
A valid Ontario Health Card or proof that you have applied for an Ontario Health Card is required. If you have applied for one, the Ministry would have provided you with paper receipt -- please use this in lieu of the actual health card.
If you do not have nor have applied for an Ontario Health Card, please contact an OHIP office and submit an application. The Kingston OHIP office is located at 1055 Princess Street, Suite 401 (near Sir John A. Macdonald Blvd., across from the Kingston Shopping Centre). Health Care Connect and Health Team rostering activities are to connect Kingston's new family doctors with patients who are currently without a family physician.
My doctor is retiring and I'm worried that I won't be able to find a new doctor - should I get a new doctor?
If you have received written notification from your doctor that he/she is retiring, then you are considered to be without an family physician. If you are concerned that your doctor may be retiring but have not received notice, please speak with your doctor and/or the clinic which you go to prior to rostering with a new physician.
